How to send website form submissions to Slack

To send form submissions to Slack, create a Slack Incoming Webhook for a channel and paste its URL into your FormSubmit form's Slack integration. Every new submission appears in the channel within seconds, with each field, file links and a button to open it.

1. Connect your form to FormSubmit

If you haven't yet, point your form at a FormSubmit endpoint — see the HTML guide.

2. Create an Incoming Webhook

1. Go to api.slack.com/apps → Create New App → From scratch.
2. Name it "Website forms" and choose your workspace.
3. Open Incoming Webhooks, switch it on, click Add New Webhook to Workspace, pick a channel (e.g. #leads) and allow.
4. Copy the webhook URL.

3. Add it to FormSubmit

Open your form → Integrations → Slack, paste the URL and save. Click Send test — a sample message appears in the channel.

What the message looks like

• Header: New submission — Contact form
• Each field as its own section (Name, Email, Message…)
• File links for uploads
• Open in FormSubmit button

Tips

• Use separate channels for different forms: #sales-leads for quote requests, #support for support tickets.
• Only submissions that pass spam filtering are posted, so your channel stays clean.
• If Slack is temporarily unavailable, FormSubmit retries automatically.
